With the full understanding of this responsibility, we process your personal data as described below and within the limits ordered by the legislation, as the "hospital" as the "Data Controller" defined within the scope of article 3 of the KVK Law (Maltepe University Faculty of Medicine Hospitals).

Your name, surname, TC identity number, passport number or temporary TC identity number, place and date of birth, marital status, gender, patient number specific to our hospital and other identification data by which we can identify you; your financial data such as your address, telephone number, e-mail address and other contact data, payment and billing information; audio and digital information that may be obtained by us from time to time by electronic or non-electronic means; If you visit our hospital, your footage of camera recordings; if you use the car park, give your license plate; your general and special personal data, especially the personal health data obtained during the execution of all medical diagnosis, examination, treatment and care services; Your data, including but not limited to your private health insurance data and Social Security Institution data, your health and identity data that you send through our website, your financial data such as payment and billing information, will be processed by our Hospital for the purpose of financing and planning health services. In addition, your personal data may also be processed when you use our call center or website to use the online services of Maltepe University Faculty of Medicine Hospitals, when you attend trainings, seminars or organizations organized by the hospital, or if you visit the internet pages or make transactions on these sites.

Your personal data will be processed in accordance with the personal data processing conditions and purposes specified in Articles 5 and 6 of the KVK Law. In addition, your personal data can be processed within the framework of the Health Services Basic Law No. 3359, the Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliates, Regulation on Private Hospitals, Regulation on the Processing of Personal Health Data and Protection of Privacy and the regulations of the Ministry of Health and other legislation, It can be transferred to the physical archives and information systems of Maltepe University Faculty of Medicine Hospitals and/or our suppliers and can be kept in both digital and physical environment.

3- METHOD AND LEGAL REASON OF PERSONAL DATA COLLECTION

Your personal data is collected in all kinds of verbal, written or electronic media for the purposes mentioned above and for the purpose of providing health services within the determined legal framework, and in this context, Maltepe University Faculty of Medicine Hospitals to fully and properly fulfill its contractual and legal obligations.

4- YOUR RIGHTS REGARDING THE PROTECTION OF PERSONAL DATA

Regarding your processed personal data, pursuant to article 11 of the KVK Law;

  • Learning whether personal data is processed, requesting information if personal data is processed
  • Accessing and requesting personal health data, learning the purpose of processing personal data and whether they are used in accordance with its purpose

In accordance with paragraph 1 of Article 13 of the KVK Law, you can send your request to our Hospital in writing or by other methods determined by the Personal Data Protection Board. Since the Personal Data Protection Board has not determined any method at this stage, you must submit your application in writing to our Hospital in accordance with the KVK Law. In this context, the channels and procedures that you will submit your application to our hospital in writing within the scope of Article 11 of the KVK Law are explained below.
